Preapproval over the phone?

i'm been getting mixed signals other pre-approval threads i've read. question is, can pre-approval on phone at&t instead of going down at&t store?

and, can code used on friday pickup @ apple store, or at&t retail stores?

also, can activate exchange plan @ apple store, or at&t store only?

need help with itunes 7.7
